1. The client has followed our recommendations for maintaining and caring for the restorations.
2. The client attends a minimum of two hygiene visits per year following completion of the treatment.
3. The restorations have not been damaged or compromised by trauma, additional treatement carried out elsewhere or by gum disease.
4. There is no outstanding balance on the account.
